Personal Record Request
If you desire a copy of your mental health records, you are required to contact me in writing for this request. You can mail to or email my practice directly. I provide your records within 14 business days. However, please note, a therapist has a right to refuse records if the therapist believes the record will be harmful to your mental health. Also, there is a fee as discussed in the policy and procedures for third-party records and paperwork. You will receive the policy and procedures upon doing your welcoming paperwork.
